In accordance with NCAA rules, I understand that I can only receive books / course materials for courses I am enrolled in and I must complete the course. If I withdraw from the class I am responsible for returning any books/materials. Furthermore, I acknowledge that this book reimbursement request applies to the semester identified and I must be receiving a book scholarship for that term as part of my athletics financial aid agreement. I confirm that this reimbursement is solely for my personal use.

Because you did not receive a book scholarship for the semester reflected above, you are not eligible to receive a reimbursement for this request.
