Health Form

When do I have to submit my health form?

You must submit the health form prior to registering for classes. The most important part of the health form for registration purposes is proof of immunization for MMR (2 doses since age 1), tetanus (within 10 years) and two doses of varicella (chicken pox) or verification that you have had the disease. You do not need to get a physical prior to enrolling at UGA because we will provide you with an athletic physical. Hopefully you have already had the required shots and you just have to get the dates documented. If your doctor has another form that provides the same information as our UGA immunization form, you may submit that instead. UGA also recommends that students living on campus get the meningitis shot even though it is NOT a required shot for registration.
