Per NCAA Bylaw 15.3.4, Reduction and Cancellation during Period of Award, I have voluntarily withdrawn from the athletic team for personal reasons. As such, by signing below, I fully understand that my athletics aid provided to me for the academic year by the University of Georgia Athletic Association may be cancelled for the remainder of this academic year and for future years.
